DOT Physicals in New Jersey: What Commercial Drivers Need to Know

Published On: June 26, 2026Categories: Occupational Health, Physicals

If you drive a commercial motor vehicle (CMV) for work, staying current on your Department of Transportation (DOT) physical isn’t optional — it’s the law. DOT physicals ensure that commercial drivers are medically fit to safely operate large vehicles on public roads, protecting both the driver and everyone else sharing the road.

Care Station Medical offers certified DOT physical examinations at multiple New Jersey locations, with walk-in availability so you can get your exam completed without lengthy scheduling delays.

WHAT IS A DOT PHYSICAL?

A DOT physical (also called a commercial driver medical examination) is a medical examination required by the Federal Motor Carrier Safety Administration (FMCSA) for commercial motor vehicle operators. It verifies that drivers meet the minimum health standards required to safely operate commercial vehicles.

According to FMCSA regulations (https://www.fmcsa.dot.gov/medical), CMV drivers must pass a DOT physical to obtain and maintain their commercial driver’s license (CDL) or to operate vehicles requiring a medical certificate. Most drivers must renew their medical certificate every 24 months, though drivers with certain medical conditions may be required to recertify more frequently.

WHO NEEDS A DOT PHYSICAL?

You are required to have a valid DOT medical certificate if you operate a commercial motor vehicle that meets any of the following criteria:

– Has a gross vehicle weight rating (GVWR) or gross combination weight rating (GCWR) of 10,001 pounds or more
– Is designed to transport 9 or more passengers (including the driver) for compensation
– Is designed to transport 16 or more passengers (including the driver) not for compensation
– Transports hazardous materials in quantities requiring placards

This applies to long-haul truck drivers, local delivery drivers, school bus operators (in certain circumstances), and many other commercial vehicle operators throughout New Jersey.

WHAT DOES A DOT PHYSICAL INCLUDE?

The FMCSA-certified medical examination is comprehensive and covers multiple body systems. Your provider will review your medical history and perform evaluations including:

Vision: You must be able to see at least 20/40 (with or without corrective lenses) in each eye, have a field of vision of at least 70 degrees in each eye, and be able to distinguish traffic signal colors.

Hearing: You must be able to perceive a forced whispered voice in the better ear at at least 5 feet with or without the use of a hearing aid.

Blood pressure and cardiovascular health: High blood pressure can affect certification status. Drivers with blood pressure readings in certain ranges may be certified for shorter periods and required to demonstrate control before receiving a full two-year certificate.

Urinalysis: A urine sample is tested for protein, blood, and glucose — indicators of diabetes and kidney disease.

Physical examination: Evaluation of the musculoskeletal system, neurological function, respiratory system, abdomen, and spine.

MEDICAL CONDITIONS THAT MAY AFFECT DOT CERTIFICATION

Certain medical conditions require closer evaluation and may affect certification. The Mayo Clinic (https://www.mayoclinic.org/healthy-lifestyle/adult-health/in-depth/sleep-apnea/art-20048065) and FMCSA guidance identify several key areas:

– Diabetes: Well-controlled type 2 diabetes may be certifiable. Insulin-dependent diabetes requires waiver consideration.
– Hypertension: Drivers must demonstrate blood pressure control to qualify for a full two-year certificate.
– Sleep apnea: Untreated obstructive sleep apnea is a disqualifying condition. Drivers using CPAP with demonstrated compliance may be certifiable.
– Seizure disorders: Generally disqualifying unless the driver has been seizure-free for a specified period.
– Vision and hearing impairments: Must meet minimum standards as outlined above.
– Heart conditions: Certain cardiac conditions require waiver consideration by the FMCSA.

If you have an existing medical condition, bring all relevant documentation and a list of current medications to your exam.

TIPS FOR A SMOOTH DOT PHYSICAL

– Bring your current list of medications and supplements
– Bring your glasses or contacts if you use them for driving
– If you use a hearing aid, bring it with you
– Bring any relevant medical records for existing conditions (blood pressure logs, specialist letters, sleep study results)
– Get a good night’s sleep before your exam and stay well-hydrated
– Avoid high-sodium meals in the days before your exam if you have borderline blood pressure
