What Is a DOT Physical?

A DOT physical (also called a CMV physical or FMCSA medical examination) is a standardized health evaluation required by the U.S. Department of Transportation for commercial drivers. The examination is designed to ensure that drivers are physically and mentally capable of safely operating large commercial vehicles on public roads.

The exam follows specific guidelines established by the FMCSA and must be performed by a medical examiner listed on the National Registry of Certified Medical Examiners (NRCME). Not every doctor or urgent care center is authorized to perform DOT physicals — the examiner must be certified and listed on the national registry.

Who Needs a DOT Physical?

You are required to have a valid medical certificate if you operate a commercial motor vehicle that meets any of the following criteria:

– Has a gross vehicle weight rating (GVWR) or gross combination weight rating of over 10,001 pounds
– Is designed to transport 16 or more passengers (including the driver)
– Is used to transport hazardous materials requiring placards under hazardous materials regulations
– Crosses state lines in interstate commerce and the vehicle weighs more than 10,001 pounds

This applies to truck drivers, bus drivers, delivery drivers in large vehicles, hazmat transporters, and other commercial operators.

How Long Is a DOT Medical Certificate Valid?

Most DOT medical certificates are valid for two years from the date of the examination. However, if you have a medical condition that requires monitoring — such as hypertension, sleep apnea, or diabetes — the examiner may issue a certificate valid for a shorter period (six months or one year) and require more frequent exams to ensure continued fitness for duty.

Drivers who fail to renew their medical certificate before it expires are no longer legally authorized to operate a CMV until they obtain a new one.

What to Expect During a DOT Physical

A DOT physical covers several key areas of health evaluation. Here’s what our examiners assess:

Vision
You must have at least 20/40 acuity in each eye with or without correction, a field of vision of at least 70 degrees in the horizontal meridian in each eye, and the ability to recognize standard traffic signal colors.

Hearing
You must be able to perceive a forced whispered voice in the better ear at not less than five feet with or without the use of a hearing aid, OR if tested using a pure tone audiometer, does not have average hearing loss in the better ear greater than 40 decibels.

Blood Pressure and Cardiovascular Health
Blood pressure and pulse rate are measured. High blood pressure can affect the length of the medical certificate issued:
– Stage 1 (140–159/90–99): Two-year certificate typically issued
– Stage 2 (160–179/100–109): One-year certificate; driver must demonstrate control before renewal
– Stage 3 (180+/110+): Cannot be certified until BP is brought under control

Urinalysis
A urine sample is collected to screen for indicators of diabetes, kidney disease, and other metabolic conditions. Note that DOT urinalysis is for medical purposes only — drug testing is a separate process governed by different DOT regulations.

Neurological Evaluation
The examiner assesses reflexes, balance, and coordination. A history of seizure disorders may disqualify a driver from operating CMVs unless specific federal exemption criteria are met.

Respiratory Health
Lung function is assessed. Conditions like severe asthma or COPD may affect certification if they impair the ability to safely operate a vehicle.

Musculoskeletal Assessment
Strength, range of motion, and the ability to safely operate vehicle controls — including emergency maneuvers — are evaluated.

Medical History Review
You’ll complete a detailed medical history form prior to the examination. Honesty is essential — misrepresenting your health history can result in disqualification and potential legal consequences.

Medical Conditions That May Affect DOT Certification

According to FMCSA regulations, certain medical conditions may disqualify a driver from receiving a medical certificate, including:

– Insulin-treated diabetes mellitus (though an exemption program exists)
– Epilepsy or seizure disorders (without a federal exemption)
– Established diagnosis of heart failure or certain cardiac conditions
– Respiratory dysfunction likely to interfere with vehicle control
– Certain vision or hearing impairments not correctable within the required range

If you have a known medical condition, talk to our provider before your exam. In some cases, documentation from your treating physician or specialist can support certification. The FMCSA also has exemption programs for certain conditions.

What to Bring to Your DOT Physical

– A valid photo ID
– Your current glasses or contact lenses (if you use them)
– Your hearing aid (if you use one)
– A complete list of medications you currently take, including dosages
– Medical records relevant to any chronic conditions (heart disease, diabetes, sleep apnea, etc.)
– Documentation of sleep apnea treatment if applicable — proof that your CPAP/BiPAP is being used is often required
– Prior medical certificates if this is a renewal
