DOT Physicals in New Jersey: What Commercial Drivers Need to Know This Fall

Published On: October 7, 2026Categories: DOT

If you drive a commercial motor vehicle (CMV) for a living in New Jersey, keeping your Department of Transportation (DOT) physical certification current isn’t optional — it’s a federal requirement. Without a valid DOT medical certificate, you can’t legally operate a commercial vehicle. And with fall being a busy season for trucking, logistics, and commercial transportation, making sure your certification is up to date is critical.

At Care Station Medical Group, we offer certified DOT physicals at our New Jersey walk-in locations in Linden, Springfield, West Orange, and Secaucus — no appointment needed, open 7 days a week.

What Is a DOT Physical Exam?

A DOT physical is a medical examination required by the Federal Motor Carrier Safety Administration (FMCSA) for commercial vehicle drivers. It verifies that a driver meets the minimum health standards necessary to safely operate a commercial motor vehicle.

The exam is performed by a licensed medical examiner who is registered on the FMCSA National Registry of Certified Medical Examiners. Care Station’s providers are registered and authorized to perform DOT physicals and issue the Medical Examiner’s Certificate (MEC).

Who Is Required to Have a DOT Physical?

Drivers must obtain a DOT physical if they operate a commercial motor vehicle that:

  • Has a gross vehicle weight rating (GVWR) of 10,001 pounds or more
  • Is designed or used to transport 9 to 15 passengers (including the driver) for compensation
  • Is designed or used to transport 16 or more passengers (with or without compensation)
  • Transports hazardous materials requiring placards

This applies to long-haul truckers, bus drivers, delivery vehicle operators, construction equipment operators, and many other commercial driving roles.

How Often Do You Need a DOT Physical?

DOT medical certificates are typically valid for up to 24 months (2 years). However, if a driver has certain medical conditions — such as high blood pressure, diabetes, or sleep apnea — the medical examiner may issue a certificate with a shorter validity period (1 year or less) and require more frequent monitoring.

Drivers should plan to renew their DOT physical before their certificate expires. Driving with an expired certificate is a federal violation and can result in being placed out of service.

What Does the DOT Physical Include?

The FMCSA-mandated DOT physical exam evaluates multiple body systems and health metrics to determine driving fitness. At Care Station, our DOT exam includes:

Medical History Review

  • Review of current medications and medical conditions
  • History of sleep disorders, seizures, diabetes, cardiovascular disease, mental health conditions, and substance use
  • Surgical history relevant to driving fitness

Physical Examination

  • Vision — Must meet 20/40 acuity with or without corrective lenses in each eye; 70° peripheral vision requirement
  • Hearing — Must perceive a forced whispered voice at 5 feet or more
  • Blood pressure and pulse — Blood pressure must be below 180/110; elevated BP may result in shorter certification periods
  • Urinalysis — Checks for indicators of diabetes, kidney disease, and other conditions (NOT a drug test)
  • Neurological evaluation — Checks reflexes and limb function
  • Musculoskeletal assessment — Range of motion and strength relevant to driving
  • Cardiovascular and respiratory systems — Heart and lung sounds
  • Abdominal exam — Checks for hernias, organomegaly, or other abnormalities

Documentation

  • Medical Examiner’s Certificate (the “wallet card” you carry)
  • MCSA-5876 form submitted to FMCSA

The FMCSA’s medical standards are publicly available and detail all requirements drivers must meet.

Common Conditions That Affect DOT Certification

Certain health conditions can complicate your DOT physical. Knowing about them in advance helps you prepare:

High Blood Pressure

Hypertension is one of the most common issues encountered in DOT physicals. The FMCSA has a tiered system:

– BP below 140/90 — 2-year certificate

– BP 140–159/90–99 (Stage 1) — 1-year certificate

– BP 160–179/100–109 (Stage 2) — 1-year certificate with treatment required

– BP 180/110 or above (Stage 3) — Disqualified until controlled

Diabetes

Drivers using insulin to manage diabetes must obtain an FMCSA exemption and are subject to additional monitoring requirements.

Sleep Apnea

Drivers with untreated sleep apnea may be temporarily disqualified. Proof of CPAP compliance is typically required for certification.

Vision or Hearing Loss

Correctible vision issues are manageable with lenses. More significant deficits may require waivers or exemptions.

Tips for Preparing for Your DOT Physical

To make your DOT physical go smoothly:

  1. Bring all current medications — including dosage and the name of the prescribing physician
  2. Bring corrective lenses if you wear glasses or contacts
  3. Know your medical history — particularly any surgeries, hospitalizations, or chronic conditions
  4. Get blood pressure under control — If you have hypertension, make sure it’s being treated and monitored before your exam
  5. Stay hydrated — You’ll need to provide a urine sample
  6. Bring your previous DOT medical certificate if you have one

The American Trucking Associations (ATA) recommends that drivers stay proactive about their health year-round — not just at renewal time — to avoid surprises at the exam.
